Eldercare's Community Care Services’ goal is: “staying home independently in a safe and supportive environment”.
Our aged care packages are designed and coordinated to meet an individual’s physical and emotional needs; allowing them to remain independently in their own homes. These services are provided by Eldercare on the Yorke Peninsula only.
We provide three packages:
CACP – Community Aged Care Package, (Low Level package).
EACH –Extended Aged Care Package (High Level package)
EACHD –Extended Aged Care Package – Dementia specific (High Level Package)
Each package is case managed by a Registered and/or Enrolled Nurse ensuring that all needs are being met for not just the client, but also the families and carers involved with that person’s care.
Eldercare employs skilled and experienced personnel to ensure a quality service to the people in our community. Our service is fully accredited through the Department of Aged and Community Services.

What is the process to get community care services with Eldercare?

at the Village on (08) 8832 2822 to discuss programs and eligibility. An ACAT assessment is a prerequisite for admission to our services.
Step 2
We will contact you following receipt of both your expression of interest and your ACCR/ACAT noting your eligibility for services. Waiting lists occur from time to time.
Fees
Eldercare Community Care Service charges fees in line with those advised by the Department of Health and Ageing.
