Eldercare’s Home Care team services the Yorke Peninsula and Copper Coast regions offering a range of services including general home support, personal care and health treatments.

Personalised care plans

We understand that your needs and priorities are unique which is why a personalised Home Care plan will be created especially for you.

Your home care plan is updated regularly.  It includes details about the services you receive and can be used as a means of communication between Home Care staff, clients and family members so you receive continuity of care.

Your Home Care adviser will discuss the support options available, and the role family and friends can play, to design the best possible package for you.

Highly qualified staff

Registered and Enrolled Nurses form a large part of Eldercare’s Home Care team. With our team of experienced professionals, we can offer additional nursing services including the administration and management of medication, wound care and dementia assessments.

Transition of care

Selecting Eldercare as your Home Care provider of choice means that you will have the opportunity to continue receiving services from Eldercare as your needs and priorities change.

If you make the decision to downsize, Eldercare offers a range of independent living units located in Maitland (The Village Units) and Stansbury (Correll Cottages). Our Retirement Living team can assist you with the process and the purchase of your new home. Home Care services are still available to retirees living in Eldercare independent living units across the Yorke Peninsula.

If you begin to require more advanced care, we also have residential aged care facilities located in Maitland (The Village), Minlaton (South Park) and Stansbury (Elanora) allowing you to remain close to home. Alternatively, Eldercare has residential aged care facilities located across the Greater Adelaide region.

Home Care package eligibility

You will need an Aged Care Assessment Team (ACAT) assessment and approval to determine your eligibility for an Eldercare Home Care package.

Levels of support

There are four levels of home care packages available:

  • Level 1 supports people with basic care needs
  • Level 2 supports people with low-level care needs
  • Level 3 supports people with intermediate care needs
  • Level 4 supports people with high-level care needs.

Your ACAT assessment will determine the level of support you can receive.

Home Care fees

To receive Home Care, your service provider will ask you to pay a basic fee of up to 17.5 per cent of the single basic age pension. If your income is over a certain amount, you will also be asked to pay an additional income-tested care fee.

Any fees and eligibility for subsidy will be discussed and agreed upon with Eldercare before you receive the relevant services.

Please note: Half of your combined income is taken into account when determining your income-tested care fee if you are a member of a couple; regardless of which partner earns the income. The value of your home, or any other assets, is not included in this assessment.

Financial hardship provisions

If you believe you will face financial hardship in paying for your care costs, you can apply to the Federal Government and ask to be considered for financial hardship assistance.

Privately-funded care

Privately-funded care is also available which bypasses the need to receive an ACAT assessment and apply for a Home Care package through the Federal Government.

There will be no change to the range of services offered and no means testing in order to access Home Care. You may find that funding your Home Care privately is more cost-effective in light of any changes to the aged care pension.

Your aged care rights

Eldercare is committed to ensuring that you and your representative understand your rights in relation to the care you receive.

These rights are outlined in the Australian Government’s Charter of Aged Care Rights and they apply when you start receiving Government-subsidised home care services.

Our responsibility to uphold the Aged Care Quality Standards

Eldercare’s home care services are accredited by the Aged Care Quality and Safety Commission who conduct regular unannounced visits.

Eldercare must demonstrate that it meets all eight Standards in order to continue to receive funding from the Australian Government.
