Hospice in the RACF project

Eldercare acknowledges that palliative care is a core and growing aspect of care services in residential aged care and has actively sought various project opportunities to build high levels of clinical expertise and organisational capability that contribute, in a sustainable manner, to optimal palliative care service provision.

Hospice in the Residential Aged Care Facility (RACF) is a pilot program over a 12-month period, that supports and enables aged care staff to provide optimal palliative and end of life care for residents within the RACF.

Hospice in the RACF is a contemporary and innovative approach to supporting residents in metropolitan Adelaide and regional South Australia RACFs with palliative care and end of life care needs.

This project was made possible through funding from the South Australian and Commonwealth Governments under the Comprehensive Palliative Care in Aged Care Project.

Eldercare is responsible for ensuring that RACFs are equipped to internally support their residents in the following domains:

  • Advance care planning
  • Escalation of care pathways
  • Specialist palliative care in-reach support
  • Education and training
  • Personal care worker traineeships
  • Grief and bereavement support
